Trademark and Copyright Infringement Claims
- Your name and email address
- Name of the intellectual property owner, if different from the claimant
- A description of the trademark or copyrighted material, including registration number, registration country, and the owning company's name and website
- The location (URL) of the allegedly infringing content
- A specific description of how the intellectual property rights are allegedly being infringed
- A statement, under penalty of perjury, that the information provided in the notice is accurate and that you are authorized to act on behalf of the intellectual property owner
- Your physical or digital signature
